New Patients

What to expect

At our initial consultation, I will be asking details about your health issues and a thorough health history. This will take about 45 minutes to one hour. I may perform relevant physical exams for the next 15 minutes. In the last 15-30 minutes, I will develop with you an individualized treatment plan and teach you about the relevance of any treatments for your health and how to do any home treatments. I will send you home with a copy of your treatment plan and often include useful handouts including home treatment methods, recipes and health information.

Please bring with you the following:

• Patient intake forms and Review of Systems forms (6 total pages) filled out in black ink.   To request the forms by postal or e-mail, please call.
• Copy of any labs you have available from the last few years
• A full list of all medications & supplements and bring the bottles in a bag with you to your visit. For refrigerated items, you can just bring in the label or write down the brand name and ingredients.

  Directions:
  Heading North on 35 E - exit on Grand Ave, turn LEFT & go up Ramsey Hill. Take a wide LEFT onto Summit Avenue, turn RIGHT on Dale, turn LEFT on University Ave, turn RIGHT on Victoria Avenue, turn RIGHT on Edmund Avenue. The Natural Path to Health is 11/2 blocks on the left.
  
  Heading South on 35 E, take I-94 East From 35 W take I-94 West Exit on Lexington Parkway, turn RIGHT on University Avenue, turn LEFT on Victoria Avenue, turn RIGHT on Edmund Avenue. The Natural Path to Health is 11/2 blocks on the left.

• Southern Minnesota Outreach Clinic - 63 Egret Lane, Elysian MN 56028
  Directions:
  From Mankato - Take Hwy 14 East, turn LEFT onto Hwy 60 North, go about 10 miles, turn LEFT onto County Road 11 (1st Street) into Elysian, go around Lake Francis, turn RIGHT onto County Rd 14 and turn RIGHT onto Egret Lane. The outreach clinic is located in the last unit on the right.
  
  From I-35, exit at the Faribault/Morristown exit, Hwy 60. Head west for 22 miles to Elysian. Turn RIGHT onto County Road 11 (1st Street) into Elysian, go around Lake Francis, turn RIGHT onto County Rd 14 and turn RIGHT onto Egret Lane. The outreach clinic is located in the last unit on the right.
